How to vacuum extend/hang comfortably without edema and blisters - A special sleeve and alternative quick-taping technique, especially for uncut guys

Intro

Nothing in this guide is a novelty and I’m sure some use the same or similar methods. I just wanted to make a guide how I do it “my way” after struggling to find something that works with a sensitive glans and foreskin.

This guide has two parts:

Why this guide? Aren’t there already a dozen video tutorials from the OGs? Short answer: Yes and no.

A few months ago I finally wanted to find a comfortable way to vacuum hang without any blisters which I’m very prone to and I also wanted to prevent any edema and pinching while being uncut.

I’ve tried different recommended vacuum cup anti-blister techniques that always failed for me.

I always had my blisters at the same spot around my urethra.

Also, taping with narrow 3M medical pore tape was to finicky and bothersome for me. I needed something that worked quicker and more reliable.

If I can’t start with lengthwork in 5 minutes, I won’t start my routine at all, so this is essential for me.

I also tried different compression devices, but I don’t feel the same uniform stretch and compared to vac hanging, they also need regular controlling, set pausing and re-oxygenation.

I just want to set and forget, so I can attach my member to my under desk pulley and continue working at my PC. This is the best way for me to keep a regular routine. Not trying to make compression devices look bad, but it’s just not for me.

Existing tailored vacuum sleeve solutions

I bought the 43 mm Middle Reliever from u/6-12_Curveball, but I was struggling to get it over my member. At the time, there wasn’t the “Super Soft” but just the “Durable” variant available, which are too hard to just stretch over my member and it was a huge struggle to fit comfortably. Maybe it’s only an issue with above than average girth. I only managed to use it once properly.

Alternatively, I hated that I’d needed to lube up, get in and clean the lube up while inside the sleeve. It’s even more bothersome when taped up, because it might get your tape wet and it starts dissolving the glue.

Maybe the new “Super Soft” (stretchy) variant would be better for me, but I had no luck as they we’re out of stock almost immediately within the next order windows. I love his products and would always recommend them if possible. But you have to be quick. As I’m from the EU, I don’t just want to order one product, but a few different ones, so it’s worth the shipping costs. Often I wasn’t fast enough to fill my shopping cart with different items and get my order through in time as something sold out.

If you don’t want to use Chinese sleeves and cut them to length like in this guide, just get his Middle Reliever and call it a day. I got two of his Combo Pump Pads and they are a work of art.

Anyways I had to come up with something myself. Granted, I got inspired by the Middle Reliever and I wanted something that was customizable and easily reproducible if something breaks. Most importantly, I wanted to be able to put it over my member in layers, without struggling with my girth, misplaced foreskin, using lube, edema and pinching while hanging.

Blister prevention

I researched and saw a few guys recommending different tapes, such as Mefix (medical fleece) or tattoo tape. I can only vouch for Mefix tape, but there should be similar tapes available. I’ve tried a different brand that looks the same, but unfortunately, the glue wasn’t water dissolvable, so I had to use oils and soap to remove the tape when I was done with my routine. So keep that in mind when trying to shop for alternatives.

I just switched back to Mefix tape and it’s much more comfortable to handle and remove afterwards. I recommend buying multiple rolls so you don’t run out of it unexpectedly.

Different sizes are available.

In this guide I use the 228 x 26 mm “XXL” or “2XL” sleeve, depending which seller you’re using. I like to buy the longest sleeves, because I’m going to cut them into my preferred size anyways and you’ll get more sleeve for you money.

You want something like this! They have a completely cylindrical shape and are semi-transparent.

Don’t buy these! Those are the crappy ones. You can identify them because of the wider pre-molded vacuum cup shape at the top. Colors are usually white and blue. They usually come in sizes “S”, “M”, and “L”.

You can also identify the good sleeves by the packaging. They come with a foam protector on the inside of the sleeve. Looks like a hotdog inside a bun :)

Important: The sleeve should have some good amount of thickness to it, that’s why I’m preferring these. They are also super soft and extremely stretchy.

Guide starts here

Step 1 - Outer sleeve preparation:

I cut my sleeves into the sizes like shown in the following pictures. Your size preference and need might differ. Determine if you need a 20 mm or 26 mm diameter sleeve for your girth. My recommendation: Order both and decide by experimenting what’s more comfortable and gives you the best grip for you setup.

I like roughly 90 mm / 3.54” length for my outer cup sleeve. This one already has a specific pre-stretched form as I’ve been using it for a few months.

A new one should look like this:

Step 2 - Cup preparation:

1. Put the outer sleeve onto the cup.

2. Roll your sleeve upwards as tightly as possible.

5.jpg acd97104ce589e8b232a1a5fc71cbd0a.png 3. Roll your sleeve onto the cup, so it looks like a donut. I found the best way to do that is by pressing and fixating the tip of the cup towards your chest while you use both hands to flip the sleeve onto the cup.

Step 3 - Inner sleeve preparation:

Cut a new piece off your sleeve. Here I’m using around 60 mm / 2.36”. You can experiment by cutting a groove to have a better fit for the dorsal side of your glans. You could also cut the the opposite side pointy shaped, so it fits over your frenulum. If you do that, start with the frenulum side and cut the dorsal side last. This is a lot of trial and error to achieve your perfect fit, but the sleeve is very stretchy and forgiving when you’re wearing it.

In this example I just use a dorsal side cut-out. You could even skip the cut-outs, but you might suffer some comfort and squeezed glans ridges that turn blue after a while when hanging/extending, because the sleeve might squish your glans slightly.

Sharp scissors are recommended.

Step 4 - Toe shield preparation (Optional):

Prepare a toe shield. I pinch a small hole in the top of the toe shield so it doesn’t shrivel itself off the glans when using vacuum later in the process. You can skip using a toe shield, but I like the compression effect, so I can get in the cup more easily and it also gives you additional protection and a comfort cushion. If your cup is slightly too big, you can also always stack a few to minimize empty space.

Step 5 - Tape preparation:

1. Cut your tape into your specific length. For me 15 cm / 5.9” per piece is optimal. I like using 4 pieces that get overlapped - so basically it’s double taped around the side and quadruple taped at the top of the glans. You’ll see how in the taping section.

2. Gently stretch the tape apart and the back foil will split from the actual fleece medical tape.

3. You can now tear the tape and foil apart. Backfoil goes into the trash, tape can be glued to the edge of your table until you have all 4 pieces ready for the next step.

Step 6 - Member preparation (Foreskin and cockrings):

Just for reference, the red “D” in the upcoming pictures is the dorsal (upper) side of your member, so you don’t lose orientation.

The yellow tape on this banana is to represent the glans and should NOT be confused with the actual taping process!

It is important that your glans is fully erect and filled with blood when taping. Your foreskin should be pulled down completely and held in place by cockrings (marked blue) at the base of your member.

Alternative to cockrings: I recommend cutting off two or three 1.5 - 3 cm / 0.6 - 1.2” long sleeves and stacking them at the base of your member for maximum rigidness.

I want to emphasize again how important a 100% erect glans is and that your foreskin should be pulled down completely when taping, else you might experience uncomfortable pinching and edema while hanging/extending! Don’t rush through this step and learn how to do it properly for your own sake.

Step 7 - Taping:

Hint: Throughout all sub-steps in the “Taping” section, always pull down your foreskin and hold it pulled down when you place your tape on the shaft and coronal ridge on either side. Your cockrings at the base of the shaft will do the most work, but you’ll need to assist with one of your hands.

Cut members: Depending on how much skin or scar tissue is left over, I would recommend on pulling the skin down anyways, but judge it yourself.

1. Start to place the first piece of tape on the dorsal side of your shaft. The length of the tape to the coronal ridge (blue) should be approx. 1.5 - 3 cm / 0.6 - 1.2”.

2. Use one of your thumbs to shove the tape under the coronal ridge at the middle of your glans, while you hold the opposite end of the tape with your other hand.

3. Continue taping over your glans, urethra and down to the frenulum on the opposite side. Make sure you use some tension to prevent the tape from wrinkling.

4. Firmly use your thumbs to push the rest of the tape under the coronal ridge around the glans.

It’s important that you put the tape inside the coronal ridge and prevent air pockets. We need uniform contact between the tape and skin. Make sure you do this all around the glans when taping. Mefix fleece tape is stretchy, so it’s pretty easy to achieve.

5. Fold the tape downwards on both sides of your glans.

6. Fold the 4 flaps inwards to the sides of the glans.

Before continuing with the next step, squish your glans and shaft gently from all sides and top to make sure that the tape has 100% contact with your skin.

970e56216f2ddd25957af16137e276ba.png 7. Place the next piece of tape onto the tip of your glans. Try to be as much in the center point as possible. That way, we achieve uniform taping length on both sides.

8. Fold down the tape. Again, prevent “air pockets” around the coronal ridge and correct them with your fingers/thumbs.

10. Fold the 4 flaps inwards to the dorsal and opposite side.

Again, before continuing with the next step, squish your glans and shaft gently from all sides and top to make sure that the tape has 100% contact with your skin and the other tape layer.

11. Use the third piece of tape and repeat steps 1 - 6.

12. Use the fourth piece of tape and repeat steps 7 - 10.

Once more, squish your glans and shaft gently from all sides and top to make sure that the tape has 100% contact with your skin and the other layers of tape.

1. Place the toe shield over your glans.

2. Roll your toe shield slightly above your coronal ridge (blue).

3. Pull down your foreskin to the base of your member and unroll your toe shield.

Step 9 - Inner sleeve

1. Place the inner sleeve under the coronal ridge (blue) and overlap it with the toe shield. Correct your placement for your cut-outs by lifting and stretching it with your fingers where you want it to be.

Step 10 - Outer sleeve and vacuum cup

1. Insert your glans into the vacuum cup. You may need to remove your cockrings and wait to get semi-rigid to fit into the cup. Make sure you get your glans completely into the cup. Preserve at least 3-4 mm of space infront of the valve, so you’re not blocking it.

2. Unroll your outer sleeve.

3. Fold the outer sleeve onto the inner sleeve for a better seal and to prevent pinching on your foreskin when hanging/extending.

4. Use a pump to create a vacuum inside the cup. Personally I don’t like to use a lot of negative pressure, just a slight vacuum of negative 3 - 5 inHg. For reference: Your glans should barely change its position when you are extending, but don’t overpump your glans if you don’t want edema. There may be some visible air pockets inside the cup, but that’s fine. Your multiple layers of tape and the toe shield should prevent blisters of any form. You can always stack multiple toe shields to decrease glans edema should your cup be too big.

5. Close/secure the cup valve. You are now ready to hang/extend!

Outro:

Like stated in the intro of this guide: I’m very prone to blisters and edema. With this setup, I can hang 8 lbs easily for 1.5 h. I haven’t tried more weight but it shouldn’t be really an issue. You also might get away with only 2 pieces of tape, but I don’t want to risk any downtime in my daily routine because I caught some blisters by trying to save a few seconds taping up.

Placing the first two pieces of tape properly is the hardest part, the third and fourth piece aren’t difficult, because you just put them over the existing tape without the need of paying attention to the foreskin and placement that much. I think it definitely outweighs the risk for blisters.
